If you are NEW to Fundamentals or it has been more than 3 MONTHS since your last visit, please select INITIAL ASSESSMENT option and NOT a treatment, as per college regulations.
NEW CLIENTS: Book Initial ASSESSMENT
NEW INJURY: Book Initial ASSESSMENT
Returning > 3 months: Book Initial ASSESSMENT
Returning < 3 months: Treatment (unless it is a new injury)
đź’ł Credit Card On File
A credit card on file is required at the time of booking to reserve your appointment via our intake form.
Your card is securely stored with our booking system and helps us keep the booking process efficient and make check-in and front desk flow as easy as possible for everyone.
Please note: Your card will not be charged at the time of booking, it is only billed for services rendered, copays, outstanding balances, cancellation/no show fees, etc.
Cancellation Policy
We require at least 24 hours’ notice for all appointment cancellations and changes.
Our clinic has waitlists of patients NEW and CURRENT patients who are waiting to get in for appointments. Providing at least 24 hours’ notice gives us enough time to contact patients on our waitlist and offer them the appointment.
Cancellations or rescheduling with **less than 24 hours’ notice may be subject to a fee, as well as no shows (as per intake form).
We appreciate your understanding and consideration of other patients who are waiting for an appointment.
